Space maintainers.
When a primary tooth is lost early, the adjacent teeth drift and the successor loses its path of eruption. A space maintainer holds the space until it is needed.
When one is indicated
Premature loss of a primary molar, where the permanent successor is still some way from erupting and adjacent teeth are likely to drift into the space.
The decision depends on which tooth was lost, the patient's age, the stage of root development of the successor, and whether there is already crowding. Radiographic assessment of the erupting tooth is what settles it.
Not every early loss needs an appliance. Where the successor is close to eruption, or where the space is not at risk, a maintainer adds a hygiene burden for no benefit.
Common designs
A band and loop is the standard unilateral appliance for a single lost primary molar — a band on one abutment with a wire loop contacting the tooth on the other side of the space. Simple, and it holds the space without resisting vertical eruption.
A lingual arch or a Nance appliance maintains arch length bilaterally where multiple teeth have been lost, using the molars as anchorage with a wire running lingually or against the palate.
A distal shoe is used where the second primary molar has been lost before the first permanent molar has erupted, guiding it into position. It is a more demanding appliance in every respect.
What each design asks for
Banded appliances need the band fitted clinically and captured in the impression, or the band size specified. A band that has not been properly seated produces an appliance that does not fit.
The record has to capture the abutment teeth completely and the space accurately, along with the opposing arch where occlusion matters.
State which teeth are being banded, which tooth the loop or arch should contact, and the appliance design you want.
Follow-up matters
A space maintainer is not a fit-and-forget appliance. Bands loosen, solder joints fracture, and the appliance can be outgrown as the dentition develops.
Review at recall, check the band margins for caries, and reassess whether the appliance is still needed. A maintainer left in place after the successor has erupted becomes an obstruction.
Patients and parents need hygiene instruction specific to the appliance, because the band margin is a caries risk in exactly the population least likely to clean it well.
Talking to parents
The appliance is going into a child who did not ask for it, and the parent is the one managing compliance and hygiene at home.
Explain what it is for, how long it is likely to stay, what to watch for — a loose band, a broken wire, soreness — and specifically how to clean around it. The band margin is a caries risk in exactly the population least likely to clean it well.
Set the expectation that it will be reviewed and eventually removed, so its presence is understood as temporary rather than permanent.
What to put on the prescription
The appliance type, the teeth involved marked on the design chart, and which tooth is being maintained against.
The patient's age and the stage of development of the successor, since both affect how long the appliance needs to last and therefore how it is built.
Whether bands are fitted and captured, or whether band selection is being left to the laboratory.
