Who we are
Fairmont Dental Lab operates from 5349 Imperial Street, Suite 100, Burnaby, British Columbia V5J 1E5. We provide laboratory services to dental professionals. This website exists for those professionals; it is not directed at patients or the general public.
Questions about this policy, or requests relating to your information, can be made by telephone on (604) 875-6055 during business hours, or in writing to the address above.
What we collect through this website
The contact form asks for a practice name and a practice email address, and optionally a doctor's name, a practice telephone number, a message, and up to four uploaded files. We use this to respond to your enquiry and, where you are a client, to handle your case.
If you upload a completed prescription form, it may contain patient identifiers. Please send only what the case requires. Prescription forms are handled as clinical records under the professional obligations that apply to a dental laboratory, and are not used for any purpose other than the case they relate to.
We do not collect payment information through this website, and we do not sell, rent or trade personal information to anyone.
How the form is processed
This website is hosted on Netlify. Form submissions are received and stored by Netlify Forms and forwarded to the laboratory. Netlify processes that data on our behalf under its own privacy terms, and may store it on servers outside Canada.
Because the form can carry patient identifiers, we recommend practices sending prescription forms routinely use the laboratory's usual case submission routes rather than the web form, and use the form for enquiries.
Analytics and tags
This website uses Google Tag Manager to load analytics. Analytics collects information about how visitors use the site — pages viewed, approximate location, device and browser type — using cookies and similar identifiers. It is used to understand which content is useful, and for nothing else.
You can prevent this by blocking cookies in your browser, by using a content-blocking extension, or by enabling the browser's do-not-track setting where the analytics respects it. The site works normally without it.
The contact page embeds a Google Map. Loading that page sends a request to Google, which may set cookies under Google's own privacy policy.
Server logs
Like any website, the hosting provider records standard server logs — the requesting IP address, the page requested, the time, and the browser's identifying string. These are retained by the host for a limited period for security and operational purposes.
How long we keep it
Enquiries submitted through the contact form are retained for as long as needed to respond and, where a working relationship follows, for the life of that relationship and any period required afterwards by professional record-keeping obligations.
Prescription forms and case records are retained in accordance with the record-keeping requirements that apply to dental laboratories in British Columbia.
Your rights
Under British Columbia's Personal Information Protection Act, you may ask what personal information we hold about you, ask that it be corrected, and ask how it has been used or disclosed. We will respond within the time the Act allows.
If you are not satisfied with our response, you may contact the Office of the Information and Privacy Commissioner for British Columbia.
Security
This website is served over HTTPS. Form data is transmitted encrypted. The laboratory limits access to submitted information to the staff who need it to respond.
No method of transmission or storage is completely secure, which is another reason we ask that patient identifiers be limited to what a case requires.
